- Permanent residency in the division of society (Kurunegala district other than Pannala divisional secretariat)

- Offering a discount of 10% for medicines when taking indoor treatments.
- Providing one day room charges free when obtaining indoor treatment by the member for 07 days continuously.
- Providing all tests and treatments free of charge at the member clinic.
- Providing a special discount of 10% for Hospital Charges at the consultative specialist service.
- Providing a discount of 20% for admission fee when children of the members into the Nurses Training School.
- Providing members with a 5% discount on purchases of medicines above Rs. 5000 and a 3% discount on purchases below Rs. 5000 from the pharmacy.

Valid National ID
A valid NIC or passport is required to verify citizenship and age
Proof of Residence
Utility bill or Grama Niladhari certificate confirming Kurunegala district residency
Dependents Coverage
Spouse and children under 21 are covered under your membership
Annual Renewal
Membership is valid for one year and renewable upon payment
